Help with everyday activities, so you can live independently at home and in your community
Community First Services and Supports (CFSS) is a Minnesota Medical Assistance program that helps people with disabilities, chronic illnesses, and older adults live as independently as possible in their own homes and communities. CFSS replaces the Personal Care Assistance (PCA) program and the Consumer Support Grant (CSG).
Through CFSS, support workers help with everyday activities, household tasks, and health-related needs. People using CFSS have more choice and control over how their support is delivered, including who provides it.
CFSS offers two service models. In the agency-provider model, an agency such as Mercy Home Care hires, trains, and manages your support workers. In the budget model, you employ your own workers with help from a financial management services provider. Everyone using CFSS also works with a consultation services provider, who helps them develop their CFSS service delivery plan.
In general, people must be eligible for Medical Assistance, live in the community rather than in an institution such as a nursing home, and be assessed as needing support. Your county, tribal nation, or health plan completes the assessment.
